Start with a 90mm length, then over time go for a 100mm, then back to stock length.

You can get angled and adjustable stems as well.

Could I suggest that back pain can often be an issue with saddle fore/aft position and the angle of the saddle, I experience back pain when my saddle is even slightly angled nose up.

Yep, what Guy said, reach may not be the answer; it might well be of course but a bike fitter would start at the beginning with saddle height, fore / aft and hip angle, back arch and then reach.

Have you flipped the stem the other way ? Huh, well there's a idea. Bike(T5a)'s been ok for up to an hour but crippling after 90mins, I've been desperate to sit up straighter. There I was looking into horrible adjustable stems when I saw this thread this afternoon. Never occurred to me to flip the stem! Also moved my saddle (Charge spoon) forward/flatter and lowered by 5mm or so, and tonight it felt so much better (although i was only out for <50 mins/13 miles). Shorter reach, more upright, yay! Not the most aerodynamic riding position, but I'll address that once I've shrunk my beer gut some more!.
